Our Mission
Momentum Health’s all-inclusive programming develops young leaders to be the Momentum for change. Currently, Momentum Health supports three Community-focused programs targeting socially isolated Youth living in the South. Over 75% of our client adult youth (18-24 Y.O.A.) population have identified as homeless or unstable housing.
Leading Momentum in innovative community health services and wellness support programs, Momentum Health also provides emergency support services to adult youth clients living or conditionally residing in TX, CA, GA, and Washington, DC. Our programs target youth communities who identify as LGBT and HIV-Positive/High-Risk Negative.
Youth developed our community-based programming focused on disenfranchised Youth who cannot receive support services.
Our History
After giving up his spot at medical school, our founder, Sammi, has had to endure several personal crises as a consequence of wanting to change the world on his terms. Enduring chronic homelessness and hunger, a firm belief in resilience and faith helped Sammi survive without a home.
Sammi used his training and experience in Infectious Disease Research to cultivate unique programming targeting homelessness, hunger, and H.I.V.
With years of hard work and earned trusted relationships, Sammi has helped grow Momentum Health Development into the leading Community Health Lab. Partnering with the greatest minds in Public Health, Momentum Health has developed the most advanced community health programs to address the needs of same-gender-loving Youth of color.
In a prayer to God, Sammi promised to commit his life’s work to ensure that one day, young Black boys won’t ever have to experience the adversities he has had to survive.
Our Programs
As we grow into 2024, we hope to continue to deliver on our promise to leverage technology resources to meet the needs of our youth community and help end barriers to equitable healthcare. Momentum Health's revolutionary programs focus on cultivating and developing bright young leaders to become the Momentum against HIV, Hunger, and homelessness.
Our Tribe-Meraki Mobile Health Network will change the world for so many youths who are the most disadvantaged in reaching adequate mental health, primary care, and nutrition resources.
